The Four Numbers That Matter
A standard semen analysis measures four core parameters. Understanding what each one means — and what can move it — is the starting point.
1. Sperm Count (Concentration)
What it measures: the number of sperm cells per milliliter of semen, and total sperm count in the ejaculate.
WHO reference ranges: Concentration ≥ 16 million/mL; total count ≥ 39 million per ejaculate.
What it means if it's low: Low count (oligozoospermia) is one of the most common findings in male factor infertility. Causes include hormonal imbalances, varicocele, heat exposure, nutritional deficiencies, and lifestyle factors. Most of these are addressable.
2. Motility
What it measures: the percentage of sperm that are moving, and whether that movement is directional (progressive motility).
WHO reference range: ≥ 42% total motility; ≥ 30% progressive motility.
What it means if it's low: Poor motility (asthenozoospermia) means sperm can't reach the egg efficiently even if count is normal. CoQ10 and L-Carnitine are the most well-studied nutritional interventions for motility — both support the mitochondrial energy production sperm need to move.
3. Morphology
What it measures: the percentage of sperm with normal shape — specifically, the head, midpiece, and tail architecture.
WHO reference range: ≥ 4% normal forms (Kruger strict criteria).
What it means if it's low: Abnormal morphology (teratozoospermia) doesn't mean fertilization is impossible. But abnormal morphology can affect the sperm's ability to penetrate the egg. Antioxidants like selenium, lycopene, zinc are most associated with morphology improvement.
4. DNA Fragmentation
What it measures: the integrity of the DNA inside the sperm cell. Often a separate, add-on test.
What it means if it's high: High DNA fragmentation index (DFI \> 25% is generally flagged) is associated with failed fertilization, early pregnancy loss, and poor IVF outcomes. It's also one of the parameters most responsive to antioxidant supplementation.
What the Numbers Don't Tell You
A semen analysis is a snapshot, not a sentence. Here's what it doesn't capture:
- It reflects what happened 74–90 days ago. The sperm being analyzed today started developing about three months ago. What you were doing then; diet, stress, heat exposure, supplement use is what you're seeing now.
- It doesn't measure everything. Reactive oxygen species, sperm epigenetics, and certain biochemical markers of sperm function aren't captured in a standard analysis.
- Numbers vary. The same man can produce meaningfully different results across different analyses. One result, especially if borderline, is not a complete picture.
